Abstract:
Objective:To summarize the data of patients with internal abdominal hernia after gastric cancer surgery,to remind general practitioners to understand and master the diagnosis and treatment of acute abdomen,and to reduce the mortality of patients with internal abdominal hernia.Methods:The clinical data of 9 patients with internal abdominal hernia after gastric cancer surgery in the Minimally Invasive Digestive Center of our hospital from October 2018 to March 2022 were retrospectively analyzed,summarizing their clinical manifestations,examination methods,and treatment plans.Results:Nine patients developed internal abdominal hernia and underwent surgical treatment,including 7 cases of Petersen hernia and 2 cases of mesenteric hiatus hernia.There were 6 males and 3 females with a median age of 65 years (range,48~76 years).There were 5 cases of radical gastrectomy,4 cases of distal subtotal gastrectomy.The interval between the occurrence of internal abdominal hernia and the operation of progastric cancer was 7~630 d (average time:217 d).Abdominal CT showed "vortex sign" in mesentery in 3 cases.Mesenteric membrane was twisted and gathered in 4 cases.All 9 patients underwent surgical exploration.Intestinal necrosis occurred in 3 cases.Mesangial hiatus was sutured and discharged smoothly after operation.Conclusion:Internal abdominal hernia is a rare postoperative complication of gastric cancer,and its incidence has increased significantly in recent years.Abdominal CT examination and active surgical treatment should be carried out as soon as possible to minimize serious complications in patients with intraperitoneal hernia.
