Abstract:
Objective:To investigate the clinicopathologic characteristics,differential diagnosis,molecular characteristics,treatment and prognosis of human papillomavirus (HPV)-independent cervical adenocarcinoma.Methods:A retrospective analysis was conducted on 9 cases of HPV-independent cervical adenocarcinoma,including clinical features,pathological features,immunophenotype,treatment and prognosis.The relevant literature was reviewed.Results:Patients were all females,aged 18~57 years old,with an average of 45 years old.There were 4 cases of cervical clear cell carcinoma and 5 cases of cervical gastric-type adenocarcinoma.3 patients with cervical clear cell carcinoma were displayed as abnormal uterine bleeding,and one young patient showed cervical vegetations.Microscopic examination showed characteristic features of cervical clear cell carcinoma,including tubular,microcystic,papillary,and solid patterns with clear cytoplasm and hyperchromatic nuclei.The nuclei are large,with prominent nucleoli and a boot-like appearance,with dense eosinophilic stroma and hyaline bodies.The immunohistochemical results was positive for HNF1-β,Napsin-A,and PAX8.The proliferation index of Ki-67 was high (60%~80%),and ER,PR,and p16 were negative.3 patients with gastric-type adenocarcinoma were manifested as vaginal discharge,and one case was found due to pelvic mass admission examination,and one case was found by physical examination.Imaging revealed cervical masses or cysts in gastric-type adenocarcinoma.Gastric-type adenocarcinoma were manifested as irregular invasive growth of neoplastic mucinous glands,beyond the position of normal endocervical glands,irregular glands often angular or sac-like,some accompanied by intraluminal papillae,often closed to thick-walled blood vessels.The desmoplastic reactions were mild or absent.The boundaries of tumor cells were clear.The cytoplasm was transparent to eosinophilic,rich in mucous.The nucleus may have atypia,and the mitotic figures were rare.Cervical gastric-type adenocarcinoma generally expressed MUC6 and CEA,p53 was mutant in 3/5 of cases,and p16 was negative.Alcian blue Periodic acid-Schiff(AB-PAS) for cervical gastric-type adenocarcinoma was positive for PAS.HPV testing was negative for all cases.Conclusion:Cervical clear cell carcinoma and gastric-type adenocarcinoma are HPV-independent cervical adenocarcinoma,and are rare in clinic.These tumors have specific histological and immunohistochemical characteristics,and the combined use of a panel of immunohistochemical markers is helpful for the diagnosis of such diseases.
