Abstract:
Objective:To investigate the influencing factors investigation of cardiovascular adverse events related to minimally invasive surgery under artificial pneumothorax in elderly patients with esophageal cancer.Methods:Clinical data of 273 elderly patients with esophageal cancer by minimally invasive surgery under artificial pneumothorax were retrospectively chosen in the period from January 2010 to June 2017 and divided into 2 groups including 21 patients with cardiovascular adverse events related to surgery as A group and 252 patients without cardiovascular adverse events related to surgery as B group.Single-factor and multi-factor analysis were used to evaluate the influencing factors of cardiovascular adverse events related to surgery.Results:21 patients had cardiovascular adverse events related to surgery in 273 patients and the incidence was 7.69%.The proportion of ASA>III grade,clinical risk factors >3,hemodynamic fluctuation amplitude≥30%,airway pressure>30 cmH2O and CVP>20 cmH2O of A group were significantly lower than B group(P<0.05).The results of Logistic regression model showed that ASA>III grade,clinical risk factors>3,hemodynamic fluctuation amplitude≥30% of basic value and CVP>20 cmH2O were all independent risk factors of cardiovascular adverse events related to surgery(P<0.05).Conclusion:The occurrence of cardiovascular adverse events in the elderly patients with esophageal cancer is closely related to ASA classification,clinical risk factors,hemodynamic fluctuation range and CVP level,which can be used as the satisfactory clinical early warning index.
