Abstract:
Objective:To analyze the different factors affecting the prognosis of patients with pN2 III colorectal cancer,and to provide reference for clinical treatment.Methods:A retrospective analysis of 185 cases of colorectal cancer patients with pN2 III stage confirmed by postoperative pathology at Yixing Hospital Affiliated to Jiangsu University from January 2008 to June 2013 was performed.All the patients were followed up by us.Log-rank test and COX proportional hazards model were used to analyse the impact of clinical factors on patient prognosis.Results:The 2-,3-,and 5-year survival rates of all the patients were 68.6%,30.8%,and 18.9%,respectively.The median disease-free survival time (DFS) was 12.4 months,and the median survival time (MST) was 26.7 months.Univariate analysis showed that age,degree of differentiation,intravascular tumor thrombus,depth of invasion,chemotherapy,postoperative CEA were correlated with prognosis of pN2 III colorectal cancer (P<0.05).Multivariate analysis showed that degree of differentiation (HR=0.567,95%CI:0.410~0.783,P=0.001),intravascular tumor thrombus (HR=0.617,95%CI:0.434~0.878,P=0.007),depth of invasion (HR=1.640,95%CI:1.331~2.022,P<0.001),chemotherapy (HR=2.488,95%CI:1.678~3.689,P<0.001),postoperative CEA (HR=0.375,95%CI:0.266~0.530,P<0.001) were independent prognostic factors in patients with pN2 III colorectal cancer (P<0.05).Conclusion:The prognosis of patients with pN2 III colorectal cancer is poor,especially in patients with low tumor differentiation,tumor thrombus in the vasculature,tumor infiltration to the full layer or more and high postoperative CEA.Chemotherapy can improve prognosis.
