Abstract:
Objective:To investigate the manifestations of non-mass-like breast lesions with calcification in dynamic contrast enhanced MR images(DCE-MRI) and the diagnostic value of DCE-MRI combined with mammography(MG) in such lesions.Methods:The characteristics of MG and MRI in 102 cases showing non-mass-like breast lesions with calcification were retrospectively reviewed.The pathological results were used as the gold standard to compare the diagnostic results of MG and DCE-MRI.Results:43 benign and 59 malignant lesions were detected by biopsy.The morphology and distribution of calcification between benign and malignant lesions was significantly different(P<0.001).The calcification of benign lesions mostly performed as single punctate(13/43,30.23%)while malignant lesions were predominantly clustered punctate(22/59,37.29%).On DCE-MRI,breast lesions with calci-fication which were suspicious benign showed non-enhancement and the suspicious malignant lesions showed non-mass-like enhancement.The TIC curves of malignant lesions were mostly performed as platform type(P<0.001).The accuracy of DCE-MRI in the diagnosis of non-mass breast calcification was higher than that in MG,and the difference was statistically significant(P<0.001).In addition,the sensitivity of DCE-MRI was significantly superior to that of MG(P=0.012) and also have higher specificity(P=0.5).MG combined with DCE-MRI significantly increased the sensitivity(P=0.001) compared with MG alone.Conclusion:Different morphology and distribution of calcification have different performance in the MRI.For MG-detected non-mass-like breast lesions of calcification,combined with DCE-MRI can significantly improve the effectiveness of clinical diagnosis.
