Abstract:
Objective:To discuss the influence factors of postoperative survival rate analysis for hepatolithiasis associated with intrahepatic cholangiocarcinoma.Methods:A retrospective analysis was performed in August 2007 to July 2014 in our hospital on the clinical data of 107 patients of hepatolithiasis with intrahepatic cholangiocarcinoma,analyzing the influence factors of the postoperative survival rate.Single factor analysis was used with binary Logistic regression model,COX regression model for multiple factors analysis.Results:Kaplan-Meier results showed 107 cases of patients with postoperative 1,3,5 year overall survival rates were 51.4%,28.0%,9.3%.R0,R1,R2 and different ways of operation and only tumor biopsies of patients survival time had significant difference(P<0.005).Single factor analysis results showed that gender,age,tumor size,tumor histological type,differentiation degree of tumor,TNM staging,intrahepatic metastasis were not correlated with postoperative survival rate(P>0.05).Lymph node metastasis,portal vein invasion,resection margin and serum CA19-9 levels were associated with postoperative survival rate(P<0.05).COX regression model multifactor analysis showed that resection margin and serum CA19-9 levels influenced patient survival time (P<0.05).Conclusion:Radical resection is the most effective treatment for hepatolithiasis associated with intrahepatic cholangiocarcinoma.During the clinical treatment we should pay special attention to resection margin and serum CA19-9 levels,the independent risk factors affecting prognosis,to improve the survival rate.
