Abstract:
Objective:To investigate the predictive value of perioperative prognostic nutritional index (PNI) and systemic immune-inflammation index (SII) on the occurrence of abdominal infection after robotic radical gastric cancer surgery,and analyze the influencing factors on the occurrence of abdominal infections after robotic radical gastric cancer surgery.Methods:Clinical data of 264 patients who attended the Department of Gastrointestinal Surgery of Gansu Provincial People's Hospital and underwent robotic radical gastric cancer surgery between June 2021 and June 2023 were retrospectively collected,and were divided into infected and non-infected groups according to whether or not abdominal infections complications occurred after the operation,and the influencing factors on the occurrence of abdominal infections after robotic radical gastric cancer surgery were screened by using single-factor and multifactorial screening,and the ROC curves were used to evaluate the predictive value of PNI,SII and the combination of the two on the occurrence of abdominal cavity infection after robotic radical gastric cancer surgery.Results:The PNI values of patients in the abdominal infection group were lower than those in the non-abdominal infection group in the preoperative,postoperative day 3,and postoperative day 5 (P<0.001,P<0.001,P=0.004),and the SII values were higher than those in the non-abdominal infection group in the postoperative day 3 and postoperative day 5 (all P<0.001).The results of the ROC curve analysis showed that the PNI in the preoperative,postoperative day 3,and postoperative day 5 alone predicted the AUC values of 0.801,0.774,and 0.693 for abdominal infections in postoperative robotic gastric cancer patients,respectively,and the AUC values of 0.825 and 0.770 for SII alone on postoperative days 3 and 5,respectively,and the AUC value of 0.904 for the further analysis of combining the preoperative PNI and postoperative SII on postoperative day 3 for the prediction of abdominal infections.The results of multifactorial Logistic regression analysis showed that surgical time ≥300 min,history of diabetes mellitus,preoperative PNI value ≤43.92,postoperative day 3 SII value >1 755.08,and postoperative hospitalization ≥7 days were associated with the occurrence of abdominal cavity infection after surgery (all P<0.05).Conclusion:Surgical time ≥300 min,history of diabetes mellitus,preoperative PNI value ≤43.92,postoperative SII value >1 755.08 on postoperative day 3,and postoperative hospitalization time ≥7 days were the independent risk factors for the occurrence of abdominal cavity infections after robotic radical gastrectomy for gastric cancer in this study.Preoperative PNI combined with postoperative day 3 SII has a good predictive value for abdominal cavity infection and is more accurate than the separate indicators.
