Abstract:
Objective:To investigate the value of high frame rate contrast-enhanced ultrasound(H-CEUS) qualitative features combined with quantitative parameters for the differential diagnosis of benign and malignant prostate lesions.Methods:Sixty patients(67 lesions in total) were enrolled from February 2022 to January 2023,who presented to our hospital of North Sichuan Medical College with suspected prostate cancer(PCa) and underwent prostate biopsy.According to the pathological results,divided into benign and malignant groups.Conventional transrectal ultrasound(TRUS) and H-CEUS were performed before biopsy,to record the basic prostate appearance,qualitative characteristics and time intensity curve to obtain quantitative analysis parameters,and compare the difference.With pathological results used as the "gold standard" to develop receiver operating characteristic curves(ROC),application of the Z test to compare the diagnostic efficacy of H-CEUS qualitative and quantitative parameters alone and combined for the diagnosis of benign and malignant prostate lesions.Results:Compared with the benign group,H-CEUS imaging in the malignant group was characterized by a greater proportion of irregular blood-supplying arterial shaped(1/33 vs 11/34),and abnormalities of alignment(3/33 vs 20/34),fast-enhancement(9/33 vs 29/34),hyper-enhancement(4/33 vs 25/34),and contrast distribution in homogeneity(9/33 vs 13/34),the differences were statistically significant(χ2 =30.41,18.37,22.96,25.72,8.06,P<0.001,<0.001,<0.001,<0.001,<0.001,=0.005).Quantitative analysis of parameters malignant than benign group contrast arrival time was earlier[(16.93±3.69)s vs (21.54±3.86)s],peak intensity was higher[(48.8±5.58)dB vs (45.77±4.42)dB],the value of intensity difference[4.87(0.87,8.03) vs -0.44(-2.22,2.35)] and intensity ratio[(1.15±0.24) vs (1.01±0.97)]were greater than those of benign,with statistically significant differences(t/U=4.24,-2.324,151,-2.535,P<0.001,=0.025,=0.004,=0.015).The ROC curve showing AUC=0.938 for the combined diagnosis of H-CEUS,best diagnostic efficacy was achieved at a cut-off value of 0.44,with Yoden’s index,sensitivity,specificity,accuracy,positive predictive value and negative predictive value of 0.750,89.29%,85.71%,87.75%,89.3% and 85.7%.Based on the NRI,the combined diagnostic on qualitative features and quantitative parameters were positively improved (P<0.05).Conclusion:The application of H-CEUS to the prostate provides differential diagnosis of benign and malignant prostate diseases by observing details of the contrast perfusion and analyzing the qualitative features,and the diagnostic efficacy of combining the imaging perfusion features with quantitative parameters is superior to that of applying alone.
