Abstract:
Objective:To evaluate the short-term efficacy and safety of combination semi pelvic prosthesis and 3D printed customized semi pelvic prosthesis in reconstructing the pelvis after pelvic tumor resection.Methods:Review and analyze the medical records of patients who underwent pelvic tumor resection and reconstruction at the second department of orthopedics at our hospital from January 2010 to April 2023.Record the surgical time,intraoperative blood loss,and bleeding volume.Through follow-up,the postoperative infection,tumor development,and imaging examination of prosthesis dislocation,fracture,and other conditions were observed.Evaluate the patient's symptoms and functional improvement through VAS score,MSTS score,and Harris score before and after surgery.Results:A total of 35 patients were included,including 22 males and 13 females.There were 16 cases of primary pelvic malignant tumors,17 cases of metastatic pelvic malignant tumors,and 2 cases of benign pelvic tumors.23 cases were treated with combination semi pelvic implants (combination group) and 12 cases were treated with 3D printed customized semi pelvic implants (customization group).The age of patients in the combination group was (50.91±13.53) years old,the surgical time was (387.39±76.07) min,intraoperative blood loss was (2 578.26±1 831.78) mL,and intraoperative blood transfusion was (10.52±8.20) U.All 23 patients were followed up.3 patients developed postoperative infections,1 patient experienced prosthetic dislocation,and 4 patients died within 6 months after surgery due to tumor progression.The customization group of patients was (42.58±19.22) years old,the surgical time was (333.75±66.92) min,intraoperative blood loss was (1 194.17±720.95) mL,and intraoperative blood transfusion was (4.54±2.97) U.All 12 patients were followed up,with 2 patients experiencing postoperative prosthesis dislocation and 2 patients dying within 6 months after surgery due to tumor progression.Conclusion:The use of 3D printed customized semi pelvic prostheses for reconstruction after pelvic tumor resection can significantly alleviate patient's pain and functional recovery is better within one year after surgery.However,further follow-up is needed for evaluate the long-term efficacy.
