Abstract:
Objective:To investigate the relationship between extracellular volume fraction(ECV) and clinicopathological characteristics for colorectal cancer(CRC) based on spectral CT.Methods:The clinicopathological data of 97 patients with colorectal cancer were retrospectively analyzed,ECV was calculated using the iodine density of the lesion and the same level of aorta or iliac artery of each case on equilibrium-phase iodine density map.According to the median of ECV as the cutoff value,the patients were divided into high ECV and low ECV patients.Measurement data with normal distribution comparison between groups was analyzed using the t test.Count data comparison between groups was conducted using the chi-square test.Measurement data with skewed distribution comparison between two groups and multiple groups was analyzed using the Mann-Whitney U test and Kruskal Wallis H test.Correlation analysis was used for variables with statistically significant differences between groups.Univariate and multivariate linear regression analyses were conducted.Receiver operating characteristic curve was performed to analyze the ECV efficacy of predicting independent influencing factors with colorectal cancer.Results:Taking the median ECV 32.0% as the cutoff value,46 patients with ECV>32.0% had high ECV and 51 cases with ECV≤32.0% had low ECV.There were significant differences in gross classification,lymphovascular tumor thrombus and perineural invasion,pathological T staging and TNM staging between the two groups(P<0.05).ECV was positively correlated with lymphovascular tumor thrombus and perineural invasion(r=0.330/0.415,all P<0.001).ECV was weakly positively correlated with pathological T staging(r=0.260,P<0.05).In the gross classification,only the difference of ECV between protruding type and ulcerative type was statistically significant.There was no statistical difference in CEA,CA199,tumor location,tumor diameter,differentiation,pathological N staging and Ki-67 expression level between the two groups(P>0.05).Results of univariate analysis showed that tumor diameter,differentiation,pathological TNM staging,lymphovascular tumor thrombus and perineural invasion were related factors for ECV in colorectal cancer patients.Multivariate analysis showed that lymphovascular tumor thrombus and perineural invasion were independent influencing factors for ECV in colorectal cancer patients(P<0.05).ECV showed better diagnostic accuracy for predicting lymphovascular tumor thrombus and perineural invasion,and the area under the ROC curve value was 0.70 and 0.76,respectively.Conclusion:ECV is an important index for preoperative evaluation of colorectal cancer.It has good predictive efficacy for the presence or absence of lymphovascular tumor thrombus and perineural invasion.
