Abstract:
Objective:To compare and analyze the positioning errors and differences in target area expansion boundaries through the study of two different fixation methods for esophageal cancer.In order to provide data reference for selecting the appropriate fixation method for esophageal cancer radiotherapy in clinical practice.Methods:A retrospective analysis of 186 patients with esophageal cancer who received radiotherapy in our center.They were randomly divided into group A (96 cases with hands drooping) and group B (90 cases with hands raised up).Cone beam CT (CBCT) was used to scan the patient before treatment.A total of 1 087 CBCT images were obtained,including head and foot SI,left and right LR,anterior and posterior AP translation direction error,coronal RTN,sagittal Pitch,and transverse Roll rotation direction error.Correlation analysis and independent sample t-test were carried out respectively,and the corresponding extension boundary value (MPTV) in the translation direction was calculated according to the formula MPTV=2.5∑+0.7σ of the outer boundary of the planned target area.Results:The Plus-minus sign in the clinical data represent the forward and backward,left and right,ascending and descending directions of the translation,and the clockwise and counterclockwise directions of the three rotations around the axis,while the absolute value only represents the size of the error value.The error difference between groups A and B in the SI direction:The t value was (SI:-5.632),the P value in the SI direction was less than 0.05,and the error difference in the RTN and Roll directions:The t value was (RTN:3.408,Roll:4.842),the P values of RTN and Roll directions were both less than 0.05,and the difference was statistically significant.Through the correlation analysis between the six dimensional direction pairwise,the SI direction of group A was low correlated with LR,AP,Pitch,and Roll directions,while the AP direction was low correlated with Pitch and Roll directions.The SI direction of group B was low correlated with AP and Pitch,the LR direction was low correlated with AP direction,and the AP direction was low correlated with RTN and Pitch direction.The outward expansion values of the SI,LR,and AP directions for groups A and B were calculated using the outward expansion boundary formula as (10.17,6.75,6.70) mm and (10.94,7.57,6.22) mm,respectively.Conclusion:The error values of group A in the SI and Roll directions are smaller than those of group B,while the error values of group B in the RTN direction are smaller than those of group A.Through the calculation of the external expansion boundary,the SI and LR of group A were reduced by 0.77 mm and 0.82 mm,respectively,and the AP direction of group B was reduced by 0.48 mm compared with group A.Both sets of fixation methods can meet clinical needs,while group A (with both hands drooping) is slightly superior.Different fixation methods can be selected according to actual situations in clinical applications.
