Abstract:
Objective:To explore the effect of body mass index(BMI) on surgery and prognosis of colorectal cancer patients.Methods:A total of 1 861 colorectal cancer patients who underwent radical surgery in our hospital between December 2010 and December 2020 were included.Patients were divided into four groups by BMI:low weight group(BMI<18.5 kg/m2),normal group(18.5≤BMI<25 kg/m2),overweight group(25≤BMI<30 kg/m2),and obese group(BMI>30 kg/m2).Basic clinicopathological data,intraoperative status,and overall survival(OS) were recorded and compared.Results:There were 122 cases(6.6%) in low weight group,1 258 cases(67.6%) in normal group,436 cases(23.4%) in overweight group and 45 cases(2.4%) in obese group.Among the basic clinicopathological parameters,BMI was only significantly associated with gender(P<0.001),and the rate of male patients increased along with increase of BMI.For intraoperative conditions,BMI was significantly associated with operation time(P<0.001) and the number of lymph nodes dissection(P=0.005).The operation time increased with the increase of BMI and the number of lymph nodes dissection decreased with the increase of BMI.Univariate analysis showed that age,tumor size,differentiation status,T stage,N stage,BMI,operation time,intraoperative blood loss and number of lymph nodes dissection were associated with the prognosis of patients(P<0.05).Multivariate analysis showed that age,differentiation status,T stage,N stage,BMI,intraoperative blood loss and number of lymph nodes dissection were independent prognostic predictors(P<0.05).Survival analysis showed that the OS of the low weight group was significantly worse than that of the other three groups(P<0.05),but there was no significant difference in OS among the other three groups(P>0.05).In the TNM stage subgroup analysis,this difference was only showed in patients with stage Ⅱ and III.Conclusion:Male colorectal cancer patients were more likely to be associated with high BMI.The higher the BMI,the more intraoperative bleeding and the fewer the lymph nodes dissected.Low BMI was an independent risk factor for poor prognosis in patients with colorectal cancer.
