Abstract:
Objective:To evaluate the short-term efficacy and safety of robot-assisted thoracoscopic surgery (RATS) versus video-assisted thoracoscopic surgery (VATS) for the treatment of lung cancer patients with different body mass indexes(BMI).Methods:Retrospective analysis of 262 minimally invasive surgically treated lung cancer patients who underwent radical lung cancer treatment by RATS (n=122) or VATS (n=140) completed by the same operator in the Department of Thoracic Surgery,Gansu Provincial Hospital from August 2019 to August 2022.Propensity score matching analysis (PSM) was performed for both groups,and then patients were divided into N-BMI group (18.5 kg/m2≤BMI<25 kg/m2) and H-BMI group (BMI≥25 kg/m2) according to their BMI.Compare the patient’s surgery-related data.Results:All patients in both groups successfully completed radical lung cancer surgery with no perioperative deaths.100 patients in each group after PSM.In terms of intraoperative bleeding,postoperative drainage time,postoperative hospital stay,number of lymph nodes dissected and number of lymph nodes dissected groups,the RATS group was more advantageous,and the difference was statistically significant (P<0.05).In terms of total postoperative chest drainage and hospital cost,the VATS group was more advantageous,and the difference was statistically significant (P<0.05).In terms of operative time,postoperative complications and intraoperative open heart,the differences between the two groups were not statistically significant (P>0.05).Subgroup analysis showed that for H-BMI group patients,the RATS group had shorter operative time and lower incidence of postoperative complications,with statistically significant differences (P<0.05).Conclusion:RATS for lung cancer offers technical and short-term efficacy advantages,but comes with the disadvantage of high costs.Lung cancer patients with high BMI are more suitable for RATS treatment,RATS can achieve better short-term outcomes and safety,especially in terms of reducing the incidence of postoperative complications.
