Abstract:
Objective:To identify the risk factors of venous thromboembolism in patients with digestive tract malignant tumor,and to explore the diagnostic value of various coagulation indexes for digestive tract malignant tumor-related venous thromboembolism.Methods:The subjects of this study were 90 patients with digestive tract malignant tumors who were treated in our hospital from July 2021 to July 2023.In the study,we divided the patients with venous thrombosis into thrombosis group (n=45),and according to the age,gender and body mass index (BMI) of the patients in the thrombosis group,propensity score matching was used to classify patients who were hospitalized and did not have venous thrombosis as non-thrombosis group (n=45).The risk factors of venous thrombosis in patients with gastrointestinal malignancies were analyzed,and the levels of thromboelastography,coagulation four indices,blood routine and other coagulation indicators[prothrombin activity (PTA),D-dimer (DD),fibrin degradation products (FDP),antithrombin (AT)] between the two groups were compared.The correlation between thrombelastogram and coagulation four indices and other coagulation indexes in patients with gastrointestinal cancer was analyzed,and the diagnostic value of receiver operating characteristic (ROC) curve for gastrointestinal cancer-related venous thromboembolism was evaluated.Results:There were significant differences in the central venous catheterization between the two groups (P<0.001).The levels of ANGLE,LY30,EPL,PT,FIB,WBC,PLT,NEU,MPV,DD and FDP in the thrombosis group were significantly higher than those in the non-thrombosis group,while the K value,TM and PTA levels were lower than those in the non-thrombosis group,and the differences were statistically significant (P<0.05).Multivariate analysis showed that LY30,FIB,MPV,central venous catheterization were the independent risk factors of gastrointestinal cancer related venous thrombosis.The Spearman correlation analysis showed that R was correlated with TT and FDP (P<0.05).MA was correlated with PT,FIB,WBC,PLT,mean platelet volume (MPV),PTA (P<0.05).K value was correlated with PTA,PT,FIB,PLT,MPV,DD and FDP (P<0.05).ANGLE was correlated with PT,FIB,PLT,MPV,DD,FDP and PTA (P<0.05).LY30,EPL and coagulation four indices and other coagulation indicators were not correlated.ROC curve analysis showed that the single indexes of ANGLE,LY30,EPL,PT,FIB,PLT,MPV and central venous catheterization had low diagnostic value in the diagnosis of gastrointestinal tumor related thrombosis,but the combined use of various indexes had higher diagnostic value and medium diagnostic value.Conclusion:LY30,FIB,MPV,central venous catheterization are the independent risk factors of gastrointestinal cancer related venous thrombosis.Thromboelastography combined with coagulation four indices and blood routine for gastrointestinal cancer patients with venous thrombosis has a high diagnostic value.
