Abstract:
Objective:To investigate the application value of shear wave elastography in quantitative assessment of tissue hardness around mass in the diagnosis of benign and malignant breast tumors.Methods:A total of 141 patients with breast tumors confirmed by surgical and pathological results from July 2019 to June 2022 were collected,including 152 masses,66 benign masses and 86 malignant masses.BI-RADS classification was performed and the elastic modulus values of mass and surounding tissues with 1 mm,2 mm and 3 mm were measured(Emax,Emean,Esd,Emin,SEmax1,SEmean1,SEsd1,SEmin1,SEmax2,SEmean2,SEsd2,SEmin2,SEmax3,SEmean3,SEsd3,SEmin3),to compare the differences among 16 groups of parameters of benign and malignant masses and their surrounding tissues.Taking pathological diagnosis as the gold standard,the receiver operating characteristic (ROC) curve was drawn,and the area under the curve (AUC) of each parameter was compared to obtain the parameter with the greatest diagnostic value.Results:The elastic modulus values of benign and malignant breast masses and their surrounding tissues were statistically significant.Emax,Emean and Esd values of malignant breast masses were higher than benign ones,and Emin malignant breast masses were lower than benign ones.The AUC values of Emax,SEmax1,SEmax2 and SEmax3 were all greater than 0.90.The AUC value of SEmax2 was 0.958,the cut-off value was more than 128.33 kPa,the sensitivity of diagnosis was 77.9%,the specificity was 85.0%,and the accuracy rate was 85.5%.Conclusion:The SEmax2 value in the tissue within 2 mm around benign and malignant breast mass has the highest diagnostic efficiency,and the sensitivity,specificity and accuracy of diagnosis are better than conventional ultrasound,which has good clinical application value.
