Abstract:
Objective:To investigate the efficacy and prognosis of stereotactic radiosurgery(SRS) alone and whole brain radiotherapy with simultaneous integrated boost(WBRT-SIB) for multiple brain metastases(≥4).Methods:A retrospective analysis was performed among 93 patients with multiple brain metastases(≥4) who were admitted to our hospital and had completed follow-up data from February 2012 to May 2017.The initial treatment for 48 cases was SRS alone,and for other 45 cases was WBRT-SIB.The therapeutic effect,neurological complications were compared between the two groups and the factors impacting prognosis were analysed.Results:The local control rate for six months were 83.3% in SRS alone group and 62.2% in WBRT-SIB group(P=0.034).The median intracranial progression free survival(iPFS) in WBRT-SIB group was significantly higher than that in SRS alone group (P=0.005).There were no significant differences in the median survival time between SRS alone group and WBRT-SIB group(P=0.492).The incidence of neurological complications in the WBRT-SIB group was higher than that in SRS group,and the difference was statistically significant(P=0.020).The univariate and Cox multivariate analyses showed that the absence of extracranial metastases,the Karnofsky performance score(KPS)>70,and the total target volume<10 cm3 were associated with improved survival(P<0.05,respectively).Conclusion:SRS alone and WBRT-SIB achieve similar treatment outcomes in patients with multiple brain metastases(≥4).SRS alone can be used as an effective and less toxic initial treatment for this multiple brain metastases(≥4).
