Abstract:
Objective:To investigate the effect of baseline geriatric nutritional risk index (GNRI) on the prognosis of elderly patients with multiple myeloma (MM).Methods:Clinical data of 107 elderly patients with MM newly diagnosed in our hospital from May 2010 to September 2022 were enrolled.The optimal cut-off value of GNRI was calculated based on the receiver operating characteristic (ROC) curve.Kaplan-Meier was used for survival analysis and survival curve was plotted.Log-rank test was used to compare the survival difference between groups.Cox proportional hazard model was used for multivariate analysis of prognosis.Subgroup survival analysis was performed respectively based on age,cytogenetic abnormalities and serum creatinine.Results:Of 107 patients,the median age was 68 (60~86) years,including 62 males (57.9%) and 45 female(42.1%).The median progress free survival (PFS) was 26 months and the median overall survival (OS) was 39 months.Based on the ROC curve,the optimal cut-off value of GNRI was 99.The difference between the low GNRI and high GNRI groups was statistically significanton median PFS (17 vs 47,P=0.014) and OS (29 vs 63,P=0.015).Multivariate analysis showed that GNRI≤99(HR 0.485,95%CI 0.253~0.930,P=0.029),high risk cytogenetic abnormalities(HR 2.062,95%CI 1.180~3.603,P=0.011)and serum creatinine>177 μmol/L(HR 2.316,95%CI 1.160~4.626,P=0.017)were independent risk factors for elderly MM patients.Further analysis in different subgroups showed that the patients who were newly diagnosed at the age of 60 to 65 years old or with serum creatinine≤177 μmol/L in low GNRI groups had shorter median OS(22 vs unavailable,P=0.002;39 vs 64,P=0.019)and median PFS(15 vs unavailable,P=0.001 9;26 vs 63,P=0.012).Notably,the patients with high risk cytogenetic abnormalities in low GNRI group had shorter PFS(15 vs 49.5,P=0.032).The prognostic value of GNRI score for patients with high risk cytogenetic abnormalities or low serum creatinine was statistically significant.Conclusion:GNRI score has potential prognosis in elderly MM patients,furthermore,the patients with GNRI≤99 indicates a shorter PFS and OS.
