Abstract:
Objective:To explore the effect of skeletal muscle fat infiltration (SMFI) diagnosed by CT skeletal muscle density (SMD) before surgery on the 30-day complications in patients with gastric cancer undergoing radical surgery based on the method of propensity score matching (PSM).Methods:A retrospective study was conducted to collect 286 patients who underwent radical gastrectomy in our hospital from January 2020 to March 2023.Quantitative CT was used to measure the cross-sectional mean radiation density (HU) of the L3 vertebral body before surgery.Patients were divided into two groups based on the presence or absence of SMFI diagnosed by SMD.1∶1 PSM was performed on the two groups of patients,and the differences in clinicopathological characteristics and 30-day complications between the two groups before and after PSM were compared.Multiple linear regression model was used to analyze the impact of SMFI after PSM on the 30-day complications in patients undergoing radical resection of gastric cancer.Results:A total of 46 pairs of SMFI and NSMFI patients were successfully matched for data analysis.Postoperative complications were classified as grade II and above in 51 patients,with an incidence of 17.83%.The incidence of severe complications was 3.85% (11/286) for grade III and above.One patient was classified as grade IV (complication death),with a comprehensive complication index (CCI) of 9.46 (4.55~14.45).After PSM,the incidence of postoperative complications and the median level of CCI in the SMFI group were significantly higher than those in the NSMFI group (all P<0.05).Multiple linear regression analysis showed that SMFI was an independent risk factor for the increase in the CCI after radical resection of gastric cancer (P<0.05).Conclusion:Preoperative skeletal muscle fat infiltration is associated with a higher incidence of postoperative complications in patients undergoing radical surgery for gastric cancer,and is an independent risk factor for increased CCI.
