Abstract:
Objective:To study the clinical application of different bladder filling states in intensity-modulated radiotherapy for cervical cancer for improving the accuracy of intensity-modulated radiotherapy for cervical cancer.Methods:A total of 90 patients with cervical cancer in Radiotherapy Center of Fujian Tumor Hospital between July 2020 to October 2021 were enrolled as the research objects,and were divided into the bladder emptying group,direct perfusion group and autonomous holding urine groups according to different bladder filling status,with 30 cases in each group.Then,CT positioning and target area delineation were performed,and the clinical target volume(CTV),planned target volume(PTV),rectum,bladder,small intestine,and femoral head volume changes were relatively analyzed between the three groups.During the implementation of radiotherapy,the displacement errors of the three group were collected for comparative analysis using CBCT scans performed weekly using Eeta's XVI imaging system.Results:The volume of the bladder and small intestine in the bladder emptying group respectively were(73.30±7.93)mL,(361.63±29.07)mL.The volume of bladder and small intestine in the direct perfusion group respectively were (348.30±20.87)mL,(302.00±28.41)mL.The volume of bladder and small intestine in the autonomous urine group respectively were(307.73±74.65)mL,(328.23±59.35)mL.There was a significant difference in bladder and small bowel volume changes between the three groups(P<0.01).There was no significant difference in CTV,PTV,rectum and femur volume changes(P>0.05).The dispersion coefficient of bladder volume respectively were 10.82%,6.00% and 24.26%,and the small intestine volume respectively were 8.04%,9.41% and 18.08%.The setting errors of three-dimensional(X,Y,Z,Rx,Ry,Rz) radiotherapy for bladder emptying group were respectively(0.20±0.11)cm,(0.23±0.15)cm,(0.18±0.11)cm,(0.97±0.71)°,(0.88±0.70)°,(0.88±0.55)°.The translation errors and the rotation errors in the direct perfusion group respectively were(0.21±0.14)cm,(0.27±0.18)cm,(0.19±0.11)cm,(1.09±0.76)°,(1.16±0.77)°,(0.74±0.60)°.The translation errors and the rotation errors in the self-holding urine group respectively were(0.26±0.15)cm,(0.36±0.25)cm,(0.32±0.20)cm,(1.65±1.10)°,(1.44±0.98)°,(0.98±0.64)°.There was a significant difference between the three groups in the Y-axis,Z-axis,Rx(pitch),Ry(yaw).There was no significant difference in the set-up error in the X and Rz(roll) direction.Conclusion:The positioning error is minimal during bladder emptying,and the small intestine illumination volume during bladder perfusion is minimal,maintaining the stable filling state of the bladder is conducive to improving the accuracy of radiotherapy in IMRT for cervical cancer.
