Abstract:
Objective:To explore the surgical results,complications and prognosis of colorectal cancer patients with different visceral obesity indexes.Methods:A total of 112 patients who underwent laparoscopic surgery for colorectal cancer in our hospital between March 2015 and December 2018 were recruited as the research subjects.According to the visceral fat area (VFA) measured by multi-slice spiral CT,the patients were divided into high VFA group (VFA≥100 cm2) and low VFA group (VFA<100 cm2).Patients' clinical data,surgical data,complications and prognosis were collected.Results:The body mass index (BMI) and waist circumference of the high VFA group were higher than those of the low VFA group,and the proportions of patients with hypertension and diabetes were higher than those in the low VFA group (P<0.05).There was no significant difference in the rate of conversion to laparotomy between the two groups (P>0.05).The operation time,intraoperative blood loss and postoperative exhaust time of the high VFA group were longer or more than those of the low VFA group (P<0.05).Pearson correlation analysis showed that VFA was positively correlated with operation time and intraoperative blood loss (r=0.396 and 0.526,P<0.05).Stepwise Logistic regression analysis showed that VFA was related to the operation time and intraoperative blood loss (P<0.05).The overall incidence rate of postoperative complications in the high VFA group was higher than that in the low VFA group (P<0.05).Logistic regression analysis showed that comorbidities,BMI,VFA levels,intraoperative blood loss and operation time were independent influencing factors of complications in patients after laparoscopic surgery for colorectal cancer (P<0.05).All the 112 patients were followed up with a median follow-up time of 42 months.Log-Rank test showed that there was no statistically significant difference in the progression-free survival (PFS) or overall survival (OS) between the high VFA group and the low VFA group (Log-Rank=3.489 and 1.747,P>0.05).Conclusion:High VFA can increase the difficulty of laparoscopic surgery for colorectal cancer,affect postoperative recovery,and increase the risk of complications,but it has no significant effect on long-term prognosis.
