Abstract:
Objective:To explore cervical cancer screening strategies suitable for postmenopausal women in areas with limited health resources.Methods:A total of 405 postmenopausal women were selected for cervical liquid-based cytology test(LBC),high-risk human papilloma virus test(HR-HPV) and P16INK4a immunocytochemical staining for all screened subjects.With colposcope guided biopsy pathology as the gold standard,the screening test of series and parallel method combined various screening technology was used to compare various screening alone orconbination to detected the sensitivity,specificity and colposcopy referral rate for≥CIN2.The area under the receiver-operating characteristic curve(ROC) was used to comprehensively compare the screening program.Results:The sensitivity,specificity,colposcopy referral rate and AUC values of LBC,HR-HPV and P16INK4a were(100%,94.26%,6.7%,0.971),(100%,85.04%,16.8%,0.925),(100%,98.25%,2.7%,0.991).There were statistically significant differences in the specificity of CIN2+ lesions detected between pairs(P<0.05).P16INK4a detection by ROC comparison was superior to HR-HPV detection(P<0.05).In combined screening,HR-HPV combined with P16INK4a had higher specificity than HR-HPV combined with LBC(P<0.05).In different shunt schemes,the specificity of HR-HPV primary screening P16INK4a shunt was higher than that of HR-HPV primary screening LBC shunt(P<0.05).Conclusion:As a rapid,simple and accurate preliminary screening method,HR-HPV detection is suitable for promotion in Xinjiang and other regions where cytologists are in shortage,technical strength is insufficient,and health resources are scarce.If economic conditions do not allow,P16INK4a immunocytochemical staining can replace HR-HPV detection as a screening method for cervical cancer in postmenopausal women.
