Abstract:
Objective:To explore the value of two-dimensional and three-dimensional speckle tracking imaging(2D/3D-STI) and real-time three-dimensional echocardiography(RT-3DE) in early detection of cardiotoxicity caused by anthracycline and to evaluate their feasibility.Methods:The patients with breast cancer were examined by echocardiography before chemotherapy,the third cycle of chemotherapy and after chemotherapy.Using the result of 3D LVEF as evaluation criteria,the clinical value of each parameter in the evaluation of cardiotoxicity was analyzed.Results:A total of 92 breast cancer patients were included in the study.The feasibility of conventional echocardiography and 2D GLS was better,while RT-3DE and 3D-STI were less feasible before and after chemotherapy.The difference of 2D LVEF in different chemotherapy cycles was not statistically significant.2D GLS,3D GLS,3D GRS and 3D GAS were significantly reduced from the end of the third cycle,and lasted until the end of chemotherapy.E/A,3D LVEF and 3D GCS decreased significantly after chemotherapy.The ROC curve showed that 3D GAS had the highest diagnostic efficacy for cardiotoxicity of anthracycline,with a cutoff value of -30.8%,sensitivity and specificity of 79% and 92% respectively.Conclusion:3D-STI can early identify subclinical cardiac insufficiency caused by anthracycline,but its feasibility is not good in some breast cancer patients,and its clinical application is limited.For such patients,2D GLS can provide more accurate information than 2D LVEF.
