Abstract:
Objective:To explore the clinical and diagnostic value of a disposable multi-functional medical specimen collection device under bronchoscopy combined with lung biopsy in different segments of bronchus and different sizes of lesions of bronchoscopically invisible peripheral lung cancer.Methods:Preliminary 138 cases of patients were selected with chest CT indicated peripheral lung and bronchoscopically invisible between October 2016 and October 2019.A total of 124 patients were diagnosed with lung cancer and included in the final study and analysis,the diagnostic results of lung biopsy combined with aspiration and lung biopsy alone in patients with different segments of bronchus and different sizes of lung cancer were analyzed.Results:In the upper lobe tip,middle lobe (or tongue segement) and lower lobe dorsal segment,there were no statistical difference between lung biopsys added aspirations with lung biopsy alone,the detection rates were respectively 22.2% vs 16.7,61.1% vs 44.4%,33.3% vs 27.8%.The positive rates of lung biopsy added aspiration with lung biopsy alone in the posterior segment of upper lobe,anterior segment of upper lobe,basal segment of lower lobe were 63.2% vs 26.3%,65.2% vs 30.4%,64.3% vs 32.1%,there were statistical difference.The positive rates of lung biopsy combined with aspiration in the tumor diameters≥3 cm(n=50) and tumor diameters 1~<3 cm(n=74) were 78.0% and 37.8%,respectively.The positive rates of lung biopsy alone were 54.0% and 13.5% respectively,showing statistical differences.And no matter the lung biopsys combined aspirations or the lung biopsys alone,there were a statistical difference between the tumor diameters≥3 cm and the tumor diameters 1~<3 cm.Conclusion:The multi-function specimen collector is useful for the diagnosis of patients with bronchoscopically invisible peripheral lung cancer located in the posterior segment of upper lobe,anterior segment of upper lobe,basal segment of lower lobe,and the tumor diameters≥ 1 cm.That could be used as a joint diagnostic method.
